The plan involves higher deductibles and copayments. HMO — You pay a flat monthly rate with no deductibles for services provided by professionals within the HMO network. All services must be cleared by the HMO. PPO — The network of medical providers is broader than an HMO and care is paid for (at a reduced rate) as it is received rather than beforehand through a set fee. POS — This plan combines features of HMOs and PPOs. You decide whether to use an in- network or out-of-network provider. Insurance companies more commonly offer high-deductible health plans that benefit both consumers and employers. These plans can include accounts such as: HSA (health savings account). Participant- provided tax-free deposits fund an account that helps offset the generally higher deductibles. HRA (health reimbursement account). The group plan or the insurance provider funds reimbursement accounts for the generally higher deductible. HRA account eligibility is usually tied to wellness or incentive programs, such as biometric screenings and health assessment surveys.
