The Episcopal Church Home has been in operation for 132 years and is affliated with the Episcopal Diocese of Kentucky. Their mission is to enhance the physical, emotional and spiritual life experience of adults as they age. The home is a 5-STAR rated community by CMS (Center for Medicare and Medicaid Services). The Episcopal Church Home offers many levels of care, encouraging people to maintain their independence. The ability to age in place can begin in Dud- ley Square's patio homes with additional options in memory care, personal care, in and out-patient rehabilitation services, skilled nursing and end of life care. The Rehab-to-Home Program at The The Episcopal Church Home — Enhancing Lives for 132 Years Episcopal Church Home is uniquely de- signed to each participant. The therapy team learns about the individual's life and routine and develops a plan of ther- apy to support what that person needs to successfully return to the living envi- ronment of their choice. They are very proud to report that 96 percent of the Rehab (short-term stay) residents return to their previous living arrangements. Residents, families and members of the community enjoy St. Luke's Chapel which is part of The Episcopal Church Home and has a specially-designed, ful- ly-accessible sanctuary. The community and St. Luke's Chapel is open to persons of all faith traditions and the spiritual needs of the residents drove its unique design. The Episcopal Church Home believes it is very important to include interaction with pets, plants and fowers and chil- dren throughout the community. They have many interactive areas and special- ized activities, including a playground. There are dogs, cats, bird aviaries and large fsh tanks throughout the building. The Episcopal Church Home is located at 7504 Westport Road, near the corner of Lyndon Lane, in eastern Metro Lou- isville, one minute from the Westport Road exit off the Watterson Expressway.
